  • 41 y.o. CF unremarkable PMH with CC of pain in the medial and plantar foot.
  • Pain present even at rest, particularly at night.

  • Onset:
    •  Off/On for 10 years, worse past few weeks, no history of trauma, no history of infection
  • Treatment:
    • NSAIDS, Cryotherapy-minimal relief
  • Aggravating factors:
    •  Prolonged standing and high heels
  • Other questions:
    • No back/sciatic pathology
    • No history of seronegative arthritis
    • No meds/supplements

Physical Exam
  • Vascular:
    •  2/4 pulses
    • No varicosities
  • Derm:
    • No lesions, cellulites, LAP/Lymphangitus
  • MS:
    • Morphology WNL, no contractures, normal arch height
    • No focal weakness
    • No intrinsic deficits
  • Neuro:
    • (+) Tinnel
    • (+) PF/INV
    • (-) Straight leg test
    • Normal dermatomes
    • Normal reflexes
